Consent to Participate in Research (Confidential Research)

Identification of Investigators & Purpose of Study
You are being asked to participate in a research study conducted by Danielle (Gallagher) Maxham and Thomas Lavenir from James Madison University.  The purpose of this study is to better understand how to design and implement an early alerts system to improve retention rates, including identifying and closing equity-based retention gaps. This study will contribute to the recommendations made by the QEP working group.  

Research Procedures 
Should you decide to participate in this research study, you will be asked to sign this consent form once all your questions have been answered to your satisfaction.  This study consists of an interview that will be administered to individual participants via a web conferencing platform like Webex or Zoom. You will be asked to provide answers to a series of questions related to early alerts, student success & retention, and equity-based retention gaps. We will record the audio and video via the web conferencing platform.  

Time Required 
Participation in this study will require 60 minutes of your time. 

Risks  
The investigators do not perceive more than minimal risks from your involvement in this study (that is, no risks beyond the risks associated with everyday life). We cannot guarantee anonymity but will ensure security of recordings and data.  

Benefits 
There are no direct or indirect benefits to interview participants. Students, faculty, and staff at James Madison University may benefit from the knowledge shared about designing and implementing an early alerts system. Furthermore, others interested in early alerts and student success & retention may also benefit in the future. 

Incentives 
Participants will be given the choice of a meal voucher valid at any JMU campus dining options, a JMU Bookstore discount, or a JMU branded item. Limited supply of each item. All participants will receive one item.

Confidentiality  
The results of this research may be presented at conferences, institutes, or published. The results of this project will be coded in such a way that the respondent’s identity will not be attached to the final form of this study. The researchers retain the right to use and publish non-identifiable data. While individual responses are confidential, aggregate data will be presented representing averages or generalizations about the responses as a whole. All data will be stored in a secure location accessible only to the researcher(s). Upon completion of the study, all information that matches up individual respondents with their answers (including audio/video recordings, if applicable) will be destroyed after five years. Final aggregate results will be made available to participants upon request. 

Participation & Withdrawal  
Your participation is entirely voluntary. You are free to choose not to participate. Should you choose to participate, you can withdraw at any time without consequences of any kind.

Giving of Consent 
I have read this consent form and I understand what is being requested of me as a participant in this study. I freely consent to participate. I have been given satisfactory answers to my questions. I certify that I am at least 18 years of age. By completing and submitting this registration survey, I am consenting to participate in this research and to be video/audio recorded during the interview.
